All services are scheduled by appointment only and in order to book with us, a Client Intake Waiver Form has to be completely filled out first with Emergency Contact information. Call or Text to schedule and we will send you the form. If you have received a special deal, provide that verification code in the message. We will then contact to schedule the appointments. List best contact number and best time to call and your preferred email address and dates that you are available. We request that you only send one email request due to high volume of calls received.
